Fees:  $110.00 per 45 minute session*
*Sliding scale adjustment available in certain cases.

Payment

Cash, check, money order, credit card (Visa & MasterCard)

Cancellation Policy

A fee will be charged for appointments cancelled with less than 24 hours notice.

Insurance

In order to use insurance, you must be evaluated and determined to have a clinical diagnosis that is billable.  Insurance companies determine actual benefits at the time claims are received. 

In cases where your insurance allows for out-of-network providers, my fee is paid upfront and you will be reimbursed by your insurance.

If you are considering using your insurance, contact and ask to speak to a representative for information regarding actual coverage, including:  number of sessions yearly, co-pay amount, and annual deductable requirements.

Privacy Issues

The privacy of your health information is important to me. I will maintain the privacy of your health information and I will not disclose your information to others unless you tell me to do so, or unless the law authorizes or requires me to do so.

A new federal law commonly known as HIPAA requires that I take additional steps to keep you informed about how I may use information that is gathered in order to provide health care services to you. As part of this process and when treatment with me is agreed upon, I am required to provide you with a written Notice of Privacy Practices and to request that you sign a written acknowledgment that you received a copy of the Notice.

The Notice describes how I may use and disclose your protected health information to carry out treatment, payment or health care operations and for other purposes that are permitted or required by law. This Notice also describes your rights regarding health information I maintain about you and a brief description of how you may exercise these rights.
